NOTICE OF PRIVACY PRACTICES
Wake Radiology’s privacy notice describes how medical information about you may be used and disclosed and how you can get access to this information. We encourage all of our patients to review this policy carefully.
WAKE RADIOLOGY’S LEGAL DUTIES
Wake Radiology is required by law to maintain the privacy of your protected health information, provide you with notice of its legal duties and privacy practices with respect to your protected health information and abide by the terms of the notice currently in effect. Wake Radiology reserves the right to change the terms of its notice and to make the new notice provisions effective for all protected health information that it maintains. When changes are made, a new Notice of Privacy Practices will be posted on this Wake Radiology Website (www.wakerad.com), in our waiting rooms, patient exam areas, and will be provided to you on your next visit. You may also request an updated copy of our Notice of Privacy Practices at any time.
USES AND DISCLOSURES OF HEALTH INFORMATION
Wake Radiology uses your protected health information for treatment; obtaining payment for treatment; and conducting its health care operations. For example, Wake Radiology will use your medical information to perform requested diagnostic or treatment services and provide your treating physician with a radiology report. We may share your medical information with your insurance company, our billing department and collection agencies. We may use your protected health information to perform quality assurance and arrange for accreditation organizations to evaluate our practice. Wake Radiology may also use or disclose your protected health information without authorization for public health purposes, for auditing purposes, and for emergencies. We also provide information when required by law. Wake Radiology may use your protected health information to contact you to provide appointment reminders or to tell you about its health-related services that might be of interest to you.
In any other situation, Wake Radiology’s policy is to obtain your written authorization before disclosing your protected health information. If you provide us with a written authorization to release your information for any reason, you may later revoke that authorization to stop future disclosures at any time.
PATIENT’S INDIVIDUAL RIGHTS
You have the right to request to inspect or obtain a copy of your protected health information. You have the right to request that we correct any inaccurate or incomplete information in your records. You also have the right to request an accounting of disclosures of your protected health information for reasons other than treatment, payment, or other health care operations. Such requests must be in writing. You have the right to reasonably request to receive confidential communications of protected health information by alternative means or at alternative locations. Such requests must be in writing. You may also request in writing that we not use or disclose your protected health information for treatment, payment and administrative purposes except when specifically authorized by you, when required by law or in emergency circumstances. Wake Radiology will consider all requests on a case-by-case basis, but the practice is not legally required to accept them. You have the right to obtain a copy of this notice upon request.
CONCERNS AND COMPLAINTS
If you are concerned that Wake Radiology may have violated your privacy rights or if you disagree with any decisions we have made regarding access or disclosure of your protected health information, please contact our practice Privacy Officer at the address listed below. You may file a complaint with the Privacy Officer. You may also send a written complaint to the Secretary of the US Department of Health and Human Services.
