WAKE RADIOLOGY UNC REX HEALTH
Wake Radiology UNC REX provides simple access to your medical and imaging records for services that were performed at any of our outpatient offices. Your results are often needed for a follow-up appointment with another physician.
Patients can log into our Patient Portal to access their medical record, results, reports and images. Our Patient Portal is available anytime. If you need a CD of your images and reports, please allow 48 hours to complete this request. We can make it available for pick-up at an office near you.

The Wake Rad UNC REX Patient Portal provides anytime, always available access to our patients imaging reports. Simply create a login to access your radiology results, reports and images. Click here to sign up for the Patient Portal.

Was your imaging provided in a hospital setting? Click here to find out who has what you need. Medical Records for services performed in a hospital setting must be requested from the hospital directly.
Release Forms
You will need to fax to Wake Radiology at 919-859-0182 or email to filmrequest@nullwakerad.com the completed release form. Please download the release form that best suites your situation.
* Please note that there is a 24 – 48 hour turn around time for all requests.
